Could be serious: If you were doing heavy upper body work, you could have nerves pinched in neck or from upper rib. Also possible you were overbreathing or your blood pressure got low. Testing is needed: This is not just a concussion, since you passed out first. This history is important. Some of the conditions that cause loss of consciousness can cause exercise intolerance. Just your hands?: There is a condition called physical or cholinergic urticaria--hives resulting from physical activities, but often else where on body as well, not just hands. They can be quite swollen (as piture here) and at times quite itchy. Exercising in cooler weather lessens it a bit. Swimming is less likely to cause hives in such case.
