Serious= Cystic?: If by serious you mean cystic, or hard nodular inflammatory component to your acne, then you will need an exfoliating acne treatment. I would probably skip otc meds and go directly to a tazorac or retin a (tretinoin) micro. These can be prescribed through a prime consult. Acne : Acne can occur on the scalp. We also sometimes call it folliculitis--inflammation (or occasionally infection) of hair follicles. Doxycycline is a commonly prescribed antibiotic for both acne and folliculitis. Another course of the same could be done and then followed by a topical antibiotic solution like clindamycin. There are no good options for otc treatment--shampoos, etc. Rarely allergic reactions to medications can present as pustules in the scalp. A medication history and exam would need to be done to exclude this possibility. ...Read more
