It sounds like you already have very little body fat, which is great, and want to put on some lean muscle, which is great. Resistance training, making friends with guys who resistance-train, and getting plenty of protein are key; if your physician is okay with it, try creatine and an electronic muscle stimulator. Look great & be healthy.

The heavy smoking however is a very great health hazard and places him at higher risk for heart disease, cancer, emphysema, stroke, etc. If he a smoker does one thing for their health it would be to quit.
