Doctor insights on:
What is the recommended pain medication for someone taking gleevec? My oncologist said tylenol (acetaminophen) is fine but everywhere online says to avoid it.
Yes.: Yes, some patients on imatinib report graying of the hair due to the drug. The good news is that CML patients taking imatinib have the possibility of living long enough to get gray hair from old age, an option not available before that drug was developed. So keep the side effect in perspective. It is safe to use hair dye while on imatinib. ...Read more
C-kit analysis: Whether patients with gist are candidates for Gleevec (imatinib) depends on the status of a particular gene called c-kit, or cd117. This is a test that must be run on the tumor biopsy tissue and is generally coordinated with the help of the pathology laboratory. In select cases, some patients are candidates for Gleevec (imatinib) regardless of their c-kit results, but this is highly individualized. ...Read more

What should I do if I have to take gleevec (imatinib) for leukemia and its hard to work because it makes me so sick?
There are a large amount of information on different approaches to manage toxicity based upon the response of the cml. It is expected that people will live for decades with CML on treatment. There are dose changes and other agents to be considered.
Don't stop the drug. Have a discussion with your oncologist about the options for you to be functional. ...Read more
No data yet...: Gleevec (imatinib) inhibits a number of tyrosine kinases in cells (c-kit, pdgfralpha, c-abl). There is very recent data that Gleevec (imatinib) may have activity in retinoblastoma, but these experiments where done on isolated retinoblastoma cells grown in culture. There are no clinical trials showing that Gleevec (imatinib) can treat retinoblastoma. Future clinical trials may be planned to address this question. ...Read more

Keep it: If you are past 3 months, then it is best to continue to taking Gleevec (imatinib). Stopping it for a few months is another option. The drug is generally safe although there is not a great deal of experience using it in pregnant women, but there have been close to 100 normal pregnancies in my limited experience. ...Read more
Noticed there is a serious interaction between flomax, which I was just prescribed, and gleevec, but none with sprycel, (dasatinib) which I take for cml. Correct?
Increase serum Floma:
Gleevec increases serum levels of flomax, and therefore dosage adjustment may be necessary.
Flomax is metabolized through cyp3a4 and sprycel (dasatinib) is also a substrate for cyp3a4, so may also increase the levels of flomax. ...Read more

What is the life expectancy of a man in good general health with cml x 6 years in remission with gleevec (imatinib) 400mgs po qd.?
Good: From the study, about 70-75% people are still being on Gleevec (imatinib) and still on complete remission for over 12 years now. In addition to gleevec, (imatinib) we have 3 other medications available- i.e. Dasatinib (sprycel), nilotinib (tasigna) and the newest one- bosutinib (bosulif) that you can use if needed. So, I will say that for a leukemia, the life expectancy in CML is actually pretty good. ...Read more

Is treatment necessary for systemic mastocytosis? I am taking gleevec (imatinib). What would happen if I stopped taking it?
D/w oncologist: There are indolent systemic masto, and agressive systemic mastocytosis (asm). Many times, with indolent disease- therapy will be aimed only to control the symptoms- itching, diarrhea etc. If you have an asm- then usually treatment is started. Asm can manifest from slowly progressive to rapid fatal disease. Organ involvement, transformation to other blood DX including acute leukemia can happen. ...Read more

No: No. I don't think so. If you have a lump in the breast- you need a mammogram and sonogram to be done. If it is confirmed to be a solid mass- a biopsy should be done to figure out what the lump is. Breast lump has nothing to do with the gleevec (imatinib). Go see you md right away so mammo and sonogram of the breast can be done as soon as possible and biopsy should also be done accordingly. ...Read more

One of the side effects of gleevec (imatinib) is bone pain. I can't take nsaids. I am on a combination of Duragesic and vicodan. Any suggestions other than narcs?
Adjuvant therapy: There is good evidence that shows anti-epileptic medications, Gabapentin and Lyrica (pregabalin) specifically, can work in conjunction with opioids in reducing pain as they work on different pain receptors. Cymbalta has also been approved for various pain conditions including back pain, osteoarthritis pain, fibromyalgia, diabetic peripheral neuropathy, etc. ...Read more

Will gleevec (imatinib) help my sister who has stage 3 breast cancer that has now gone into bone cancer stage 4?
NOT STANDARD OF CARE: Gleevec is indicated for gist and cml. In breast cancer it has been looked at in preclinical studies and phase ii studies. Although it has been suggested that Gleevec might help in bone metastasis by inhibiting osteoblast activity I would only use it under a clinical trial. For bone metastasis there are 2 meds that help prevent complications: zometa & xgeva (denosumab). Talk to your doctor. ...Read more
