Wow. This is frustrating for you I am sure. I wish you were here so i can go over all of this one problem at a time. I worried you have a prostate infection extending into the bladder. The back pain is usually a sign that you need appropriate antibiotics.
Rash is often fungal or "jock itch". But, staph infections can also infect this area when there is sores.

Possible yeast: You could have a candidal infection that could be helped via otc creams. Ie lotrimin (clotrimazole) or monistat. Possible a contact dermatitis due to condom. Highly unlikely: Bowen's disease is type of very early skin cancer and extremely rare in circumcised men. Usually starts as a small well defined reddish patch with irregular border & crusting or scaling on surface. Can be caused by HPV infection or in patients with HIV/Aids.
