I'd get this checked: You're describing a lot of symptoms that are tough to put together in this forum. I'd strongly suggest you get your doctor to check you out tomorrow. I worried you have a prostate infection extending into the bladder. The back pain is usually a sign that you need appropriate antibiotics. Rash is often fungal or "jock itch". See a dermatologist: Lentigo Simplex and Penile Lentiginosis present as flat brown to black spots on the penis with irregular borders. They are not malignant BUT Lentigo Maligna(Hutchinson melanotic freckle) and melanoma may look similar to Lentigo Simplex. Small clear: Fluid 'blister' may be herpes simplex. Your best bet is to confirm a diagnosis, get HIV and syphilis testing, and use a condom. I'd see a urologist or dermatologist or both.
