The major national laboratories have collection stations in most cities. Contact the lab for the local draw station and get your blood collected at that facility.

If it's worse with exertion it certainly could be simple vertigo but you mentioned that it's worse when you exert yourself. You need to see a doctor, sooner rather than later, have an EKG done and a good exam with more detailed history.

Can be helped: Your description raises questions of a migraine etiology to explain most of your issues, but other explanations could be considered.
