Galactorrhea is a condition where milk flows from the breast at a time not associated with pregnancy, childbirth, or nursing. It can occur at any age and in both sexes. While it might be due to innocent benign processes and even medications/drugs, it needs evaluation to rule out potentially serious causes.

How is galactorrhea typically treated?

Depending on cause: Galactorrhea has to be first evaluated thoroughly to diagnose the underlying cause. Once identified, your doctor may target their treatment to that cause. In most cases, medical treatment is adequate and in some cases surgical treatment may be necessary.

Big prolactin: Hi. Why was prolactin checked? If you have regular periods and no breast milk production, have your doc check "big prolactin" or "macroprolactinemia"; the test is commercially available. Big prolactin is a complex of prolactin molecules in the circulation that does NOT cause galactorrhea/amenorrhea syndrome. It does NOT impair fertility, and does NOT need treatment.

Get their attention!: Speak directly, loud and clear, placing yourself in front of them, demand that they look into your eyes! Have them breathe slow and deep while looking at you. You may have to demonstrate this for them; it may help to have a hand or two on their shoulders. The object is to get them grounded in the present moment. One way is to have them mimic exactly what you are doing. Try to keep their eyes open!

Yes: Mary there can be many reasons for your observation. You have had somewhat of a work up but would stop squeezing and if it persists check with a good Internal medicine doctor or endocrinologist

Hormonal imbalance: Galactorrhea (breast milk production when not pregnant) can be caused by multiple events. Although our first thought is a pituitary (brain) overproduction of prolactin, frequently the cause can be be a thyroid disorder, side-effect of medication (anti-depressants, birth control pills), the result of scarring or injury to the chest wall, or even a side effect of marijuana use.

Good Question: Sounds like you have done some research. You need blood tests and a possible check of your pituitary gland. I would start with a gyn or endocrinologist or internist for the workup.

Good Question!: Knowing the cause of the galactorrhea determines how the condition is treated. Often it can be a simple as medication change. Psychotherapeutic drugs and GI drugs are often the simple culprit. Once cause is determined, the prolactin levels can be measured while the symptoms abate.

By symptoms: Galactorrhea is diagnosed by symptoms. It is usually due to an elevation in your prolactin level, a hormone secreted from the pituitary gland. Prolactin can be elevated due to a tumor or by excessive nipple stimulation. Pituitary gland is located between the optic chiasm in the brain. If galactorrhea is due to a pituitary gland tumor, patient may have visual changes and/or headaches.

No: It is merely bothersome. However the cause should be evaluated.

Yes, possibly: Injury to the pituitary stalk can potentially lead to galactorrhea (case reports available), although the impact would likely affect other hormones as well, leading to e.g. Diabetes insipidus. If I had a patient with post-traumatic galactorrhea i'd be suspicious of a pre-existing pituitary prolactinoma and get prolactin levels and brain MRI to rule out such a lesion. Treatment - medical first.

No and yes: Amenorrhea can happen after stopping pills but should not last more than 6 months. Galactorrhea is not. You should consider seeing a gyn, or a medical or reproductive endocrinologist.

Not much: Other than making a mess in your bra this is typically a benign condition.

It means: You have increased your prolactin, which can lead to weight gain, abnormal periods, etc. Over long term, but that is generally only with much higher elevations than you can achieve with nipple stimulation. Enjoy.

Hello Dr, I think I have hyperprolactinemia. When to check my prolactin level in my cycle day?? have galactorrhea. I am in ttc, do affect fertility?

Yes: It does but firstly what is your prolactin level. Depending on the level there are many causes of hyperprolactinemia including psychotropic meds, thyroid and renal disease. If prolactin is greater than 200 will probably require MRI to rule out Prolactinoma
