Could be: Although it could be fungus, the loose nail must come off. To leave it attached can cause pain and be a source for infection. See your doctor for a referral to a specialist to remove the loose part of the nail. It should grow back after removal. ...Read more

Yeast seems unlikely: This may be possible, but I doubt yeast is the most likely explanation. Some other infection or inflammatory bowel disease may be more likely. If you are the receptive partner in anal sex, and if at risk for STDs, gonorrhea or chlamydia are possible, or possibly herpes but less likely without substantial pain. Rectal cancer could do it. Green stool: Occasional green stool color is normal, usually due to passage through the digestive tract with some degree of rapidity such that bile is incompletely metabolized (which contributes the brown color). Unlikely: A high ph is alkaline, not acid. Any diarrhea can cause diaper rash. There are many causes of diarrhea at this age - infection, malabsorption, celiac etc. Please follow up with your doctor; see a ped GI specialist if recommended. Meanwhile clean with soft cloth and soap/water, and use appropriate antibiotic or antifungal ointment for the rash. Lactose intolerance is uncommon under age 6. ...Read more

If this affects your lung, then one needs to rule out the presence of fungal ball in the lung also. With the large spleen, I suspect that you are having a systemic disease and this requires tests and examination to clarify.
