Elderly male sleeping up to 16 hrs/day, misses out on breakfast, lunch. Usually eats a lot of food at dinner, but still losing weight. Reasons?
Inadequate nutrition: When evaluating weight loss, we start w/basics: Can they afford food? Is that person getting enough to eat? Eating a lot of food at dinner may not make up for missed calories from 2 meals. We also want to assess ability to chew & swallow food. In this case, I'd also want to find out why he's sleeping 16hrs/d, which is twice avg am't. After that, then we look for hyperthyroidism, cancer & other dz

Poor nutrition in: The diet you mention, not the amount that is the real issue. Please see a nutritionist who can help you create a good food plan. Every meal needs to contain protein, fats, and carbs. Lots of good fats helpful to the brain. These are things like olive oil, coconut oil. Check w/doc first. Peace and good health.
Almost anything: Vitamin d is responsible for many things in all our body cells. It is now regarded as the commonest vitamin deficiency and is really a hormone made in the skin from sunlight exposure. We are seeing many diseases related to it. It is no longer "just a bone nutrient" and the blood level of d3 should be between 60 and 80.
Consult a doctor: Weight gain in the elderly can signifiy something medically wrong or even a medication side effect. It is important to have medical evaluation to determine correct diagnosis and approach. Hopefully this will provide answers and some steps to take.
Anything but fiber: By definition, fiber is indigestible. However, it's still necessary for all of us to maintain proper bowel function. While there isn't any specific best food, there are plenty to avoid or at least consume minimally or in moderation eg fast, fried, snack, salty etc. Regardless of age, we still need to eat fruits & vegetables, protein of some sort plus whole grains, dairy etc. Let's not forget H2O
Evaluate smell: The loss of the ability to taste may decline slightly as we age- perhaps explaining why some elders like more sweets or salty things. However, the decrease in smell may also have something to do with it. Hav your senior evaluated by an ENT. Other more serious conditions may include brain pathology including tumors.

Extremely common: Cardiovascular disease, which means diseases of the heart and blood vessels, is the most frequent cause of death among elderly people. This includes disease of the coronary arteries, heart failure, strokes, blood clots, etc.
Perineum: Care of the perineal area should focus on maintaining a clean and dry skin with prompt attention to fecal and urine contamination. Usually soap and water should suffice for cleaning. Application of a barrier ontment is sometimes necessary in elders with incontinence. Incontinence pads or diapers are not a good answer.
Deterioration: Vision and hearing deteriorate with age. Bones get weaker. Balance and gait (walking) can worsen. Kidneys might not work as well, the list is extensive! It is important to get regular followup with a doctor especially as you get older. Get your eyes/hearing checked. Make sure you get your immunizations and any age-appropriate cancer screenings. Take calcium and vitamin d to strength bones, etc.
