Could be hernia.: A hernia could exist without a discernible bulge. The other symptoms are consistent with a hernia. Area of pain: often details the reason. Knee, ankle pain would obviously imply a joint problem. Pain/fatigue in the calves that increases with increasing exercise implies a circulatory problem. Pain with swelling in one lower leg is actually an emergency since the circulatory problem can be a blood clot that could come loose and go to the lung. Probably not: Dull aches in left arm (often with numbness, shortness of breath, sweating, post exertional) could bea cardiac symptom due to similar "wiring" between nerves going to heart and arm. These symptoms usually start centrally and radiate to the distal arm.
