Can't Say: If this is a painless lump that is not growing and you noticed it incidentally than likely ok to watch. If painful or growing or any other signs of activity, better have it evaluated. May be AC joint pain: The ac joint is where your clavicle meets your acromion (see diagram) and may become arthritic and cause a painful lump on the top of your shoulder. Depends: Depending on the location, this could be a lipoma or fatty tumor or could represent a ganglion off the ac joint which is more anteriorly based towards end of collarbone.
