Its possible: 14% of patients who present with postmenopausal bleeding will have endometrial cancer so an endometrial biopsy is warranted. Need to see images: Based on the description you gave, I would need to see the images. It certainly could be some minimal scarring in the right upper lobe & nothing to worry about. It could also be some fluid there, which if that's that case, it would need to be sampled more than likely. The pleural thickening could be related to the fibrosis, or potentially another cause. These are all normal.
1. No myometrial lesions means no fibroids or adenomysis or anything worse.

No Worries: This is an incidental finding, doesn't really mean anything. If your doctor is concerned about it he will follow it with serial ultrasounds. If you are in pain, this would push me to recommend acting surgically sooner rather than later. At 60, you should have no accumulation of blood and very little clear fluid or mucous. PCOS: Most with PCOS have the effects of estrogen increase (without production of progesterone with ovulation) which make the endometrium "thicker". Without ovulation induction, those with PCOS will have minimal success in becoming pregnant. It's hard to answer this question without knowing exactly what the ultrasound report said. It's entirely normal for a woman to have a small amount of fluid in the pelvis around the time of ovulation. A large amount of fluid, or fluid in a woman who is post-menopausal, would be unusual. What is thickened? The endometrial stripe, which represents the linig of the uterus, changes its thickness dramatically during the menstrual cycle, so this may also be totally normal. An extremely thick endometrium could represent something called endometrial hyperplasia or adenomyosis, though these words would probably show up very specifically in your report.

Thick uterine lining: At age 33, a woman not on oral (low dose) contraceptives produces a lot more estrogen as a function of increased stimulation of the ovaries to produce ovulation. This can cause increased growth of other estrogen sensitive areas. It is unlikely to be a malignancy at this age. Possible resolution: If there was a few days or more between tests, it is possible that whatever lesion that was seen on ultrasound resolved on its own. Ultrasound uterus and ovaries is sometimes a challenge based on intestinal issues and potentially what was seen on ultrasound could have been shadows of bowel.
