Vaginal fissure: Are not related to anal fissure. Anal caused by the sphincter too right and tearing when you have bm. Sphincterotomy can be effective, but would reccomend stool softness and topical nitrate creams first, followed by Botox injections if that doesn't work. I just don't understand. 2 1/2 weeks after anal fissure surgery and still feel like fissure pain! After bm burns for hours just like before surgery.

These can be difficult problems to fix and often poorly respond to treatment including surgery. Patients that have failed conservative treatment you may consider the following options:
1. L5-s1 disk with annular tissue - recommend discogram to determine if this is the cause of the pain followed by either endoscopic discectomy or fusion surgery. Please see my health guides on endoscopic disk surgery.
2. Si joint pain - consideration for si joint fusion.
3.Coccyx pain - if pain is related to prior fracture sometimes surgical removal of the coccyx may give pain relief
