Not easy: There are some prescription topical ointments (nitroglycerin/diltiazem) that can be tried but doesn't cure the condition. Also, sometimes Botox is injected but also not a permanent fix. If all above fails, surgery is indicated. Good luck. ...Read more

Vaginal fissure: Are not related to anal fissure. Anal caused by the sphincter too right and tearing when you have bm. Sphincterotomy can be effective, but would reccomend stool softness and topical nitrate creams first, followed by Botox injections if that doesn't work. Sphincterotomy is last measure because of risk of incontinence. Vaginal tears are caused by trauma, especially if there is little lubricants. ...Read more

These: These can be difficult problems to fix and often poorly respond to treatment including surgery. Patients that have failed conservative treatment you may consider the following options:

1. L5-s1 disk with annular tissue - recommend discogram to determine if this is the cause of the pain followed by either endoscopic discectomy or fusion surgery. Please see my health guides on endoscopic disk surgery.
2. Si joint pain - consideration for si joint fusion.
3.Coccyx pain - if pain is related to prior fracture sometimes surgical removal of the coccyx may give pain relief
