Due for an illness: I don't accept fever >101 as teething & am not sure it causes much stress in most infants. By 4mo the protective antibodies passed to baby during pregnancy have warn off & a kid is due for some random viral illnesses. Could be infecion: Crying causes increase pressure in ears, lound noise causes eardrum to vibrate etc. Thus can increase pain. If there is an infection, the drum area is more sensitive to pressure and thus things that cause changes here can increase pain, hence child is pulling and crying. I would have him evaluated.
