A concussion is simply traumatic brain injury as the result to a direct or indirect blow to the head. A ct can rule out fracture or bleeding but cannot diagnose a concussion. A contussion is merely the outer swelling/bruising of the skin and head.
Symptoms of concussion include: altered level of consciousness, headache, nasea/vomiting, etc.

Not enough info: My crystal ball is in the shop getting fixed. There's no good way to predict how close someone is to the end just b/c s/he fell. On the otherhand, we recommend hospice for someone who doesn't eat, can't talk, is bowel/ladder incontinent, bedbound w/pressure ulcers, assuming there's no other explanation. I realize there is no fun in taking child to ER. You only have one chance. Child could also be taken to urgicenter. Child may well not need xrays as many times they do not. Would urge that you take the child in and have hims checked out You will still need to watch him all night.
