10 swollen lymph nodes bilaterally in neck for 3 years. Painless; immobile, soft, rubbery about 1 to 2 cm. Normal chest xray. Fatigue is only symptom.

Unclear: A tender lymph node and fatigue may suggest mononucleosis, but that is only one possibility; pain under the left rib cage may be related to spleen which is not inconsistent with mono.
