Possible migraine: If you are seeing these spots and sparks in a pattern and for some defined period of time (15-30 min) this could be due to migraine visual phenomena. If it is just a momentary thing then it probably relates to the vitreous gel in one or both eyes getting a bit more liquified. ...Read more

Full eye exam/MRI: Many things can cause this. Typically black spots are "floaters" or problems with the vitreous fluid in your eye. However, pulsating can be a problem with the optic nerve and vasculature of your eye. Given your LP "pressure of 30" you need a pupil exam, a full exam of your retina and optic nerve, and likely visual field testing with OCT testing of your optic nerves, CT/MRI scan brain ...Read more
