Possible: Palpating the epididymis may be more pronounced on one side compared to the other. Palpating the epididymis in general may be difficult. Ultrasound?: There is no way of diagnosing this over the internet, sorry. You could get a recurrent epididymitis, but it would be unlikely to resolve on it's own. You may also have a varicocoele.
