Epididymo-orchitis.: You are correct in understanding that the epididymis is an appendage of the testicle. However, they are connected, and infections of the epididymis (epididymitis) can lead to infection or at least inflammation of the testicle (orchitis). This inflammatory response causes pain, fullness, swelling, and warmth in the scrotum. He needs evaluation to make sure he is getting appropriately treated.

Possible: Palpating the epididymis may be more pronounced on one side compared to the other. Palpating the epididymis in general may be difficult. If you have had inflammation/infection of one, it may get more pronounced and then more easily palpable at future visits, etc. If any concern for masses, etc, and ultrasound of the testes/scrotum would assess the anatomy in question. Do mild epididymitis cause sperms to be obstructed? Also, how do I tell if my epididymis have been damaged?

Probably not: It usually takes severe or repeated epididymitis to cause obstruction or significant damage. Mild epididymitis usually does not cause damage, but it can be annoying and hard to clear up. I recommend having it examined and treated. Good Luck!

Ultrasound?: There is no way of diagnosing this over the internet, sorry. You could get a recurrent epididymitis, but it would be unlikely to resolve on it's own. You may also have a varicocoele. This is a group of varicose veins in the scrotum. The only way to solve the issue would be an exam and ultrasound if needed.

Infection: It sounds like you may have epididymitis. This is an infection or inflammation in the epididymus (the part of the testicle that holds sperm). This is basically the male version of a uti, which is much less common in men than women. This is definitely worth a visit to the doc for proper treatment.

This: Needs an IN PERSON EXAM for the answer...your age is not included but ANY mass in the scrotum needs serious attention

See below: Usually in cases of true infectious epididymitis, the pain and swelling will resolve for the most part during or after treatment with antibiotics, although residual soreness may persist for several more days or even weeks. In rare instances, chronic pain may develop. If your pain has persisted for more than 3 months, you need to consult an urologist.

Most likely, yes.: You should go to a competent urologist to determine if this is of infectious origin, and requires proper antibiotic treatment, or of some other physical obstruction. But, do this ASAP! Surprised that you have endured this for several years with no definitive diagnosis! You need to see a proper urologist who can resolve this problem for you!

Swelling: Swelling can occur with epididymyitis. If you have persistent swelling, redness, if you get severe pain, fever, or vomiting then you must be seen as an emergency.

All part of the same: Acute epididymitis is usually caused by bacteria and can also effect the penis. You need to continue antibiotics and need to follow up with your doctor or a urologist.

Reevaluation: You should be on antibiotics. If you are and you are getting worse so that the infection has spread to your penis, then you need to be evaluated. Inflammation of the skin is termed cellulitis, and should be controlled as soon as possible. Go back to the er first since they will be able to document any changes and contact an appropriate specialist.
