Couple reasons: Normally at 6 weeks you should see a sac, a yolk sac, the fetal pole and an early heartbeat. When you don't see what you expect, there are 3 possibilities: the pregnancy is not quite as far as you think, the pregnancy is abnormal, or it is just hard to see on ultrasound. Possibily: When you are this early in pregnancy, things may seem abnormal, but end up being perfectly fine, given time. HCG levels "normally" increase by 66% every 48 hours. However, nothing is written in stone, and I have seen less then normal increases, and not so normal ultrasounds produce normal pregnancies. Without seeing the ultrasounds it is a little confusing. At the first U/S, what was measuring 6wk? > the sac or the fetus, if they saw one. The second U/S would actually be reassuring; they measured a fetus at 6w6d (the difference from LMP would just mean your dates are off), but a fetal heart rate of 154 is reassuring. Depends: If you are less than 4 weeks along, it may be too early to see anything in the uterus. But of you are 5 or 6 weeks along there should be a pregnancy very clearly seen in the uterus.
