Usually not: Usually Effexor (venlafaxine) gives people energy which is why i is recommended in the am. This could be continued depression. It could also be issues with thyroid or vitamin mineral deficiencies. Take a multivitamin every am. Ea a well-balanced healthy diet and drink 8 glasses of water a day. NIghttime jerking: In the clinical trials with effexor, (venlafaxine) a small group (1%-3%) of patients had twitching muscles. Some (5%) had tremor. Some also had hyper-reflexia and CNS stimulation -- which could contribute to what you're experiencing. Did you have a strong startle reflex before starting the medicine? If you don't need : An antipsychotic, then you could try trazodone. Start on a low dose under doctor's supervision and then gradually increase by 50 mg a night until you are sleeping adequately but not hungover the next day. ...Read more

How's your sleep: Hygiene? Retire & rise same time each day. Bedroom should be dark & cool — used only for sex & sleep. Turn off tv. No naps. Exercise regularly but not in late evening. No caffeine for 6 hrs & no alcohol or tobacco w/i 2 hrs of sleep. Dinner should be moderate sized & finished at least 2 to 3 hours before sleep. More data needed: Effexor (venlafaxine) xr works on more neurotransmitters than prozac. Also, depending upon the dose of the Effexor (venlafaxine) xr perhaps it was tapered too fast and or the dose of Prozac was not high enough. In either event you need to discuss it with you doc. Often there are other temporary medications that can be taken prn
to get you through the switch over phase. Also, the time of day that you take the meds

You may be more susceptible if you have a heart condition called congenital long QT syndrome, other cardiac diseases, conduction abnormalities, or electrolyte problem (e.g., magnesium or potassium loss due to severe or prolonged diarrhea / vomiting). However, risk is small (RARE = 0.1%) & many pts can & do take both safely. ...Read more

Panic not resolved!: I'm sorry to hear u've been struggling with panic d/o after what sounds like a long period without sxs at all. I'd talk with your doc. Has anything in your life changed recently? Are you under more stress? A psychotherapy could help here. A tradtional anxiolytic SSRI might be the route your doc eventually goes. Depression: Good question and good for you. People respond idiosyncratically to meds for anx/dep. So there are still a lot of options open and I see this time and time that if you can be patient and work with a psych prescriber slowly and methodically typically you find something that works.
