Heart attack: What you described is acute pulmonary edema and cardiogenic shock. This is caused by a massive acute heart attack where the heart suddenly stop pumping, fluid rapidly accumulates in the lungs and blood pressure falls as cardiac output drops. This is the most dreaded complication of a heart attack and unfortunately it is frequently fatal.

It: It is likely that this was just a reflex, an event that occurs as the body is not able to coordinate voluntary activities anymore and basic activities take place. Given the extent of time that the CPR took, it is likely that he was not aware of what was happening and was no longer in any pain or discomfort.

During: During CPR it is not unusual to increase pressure at particular areas of the body, and small blood vessels on the surface of the skin, or scalp, can easily burst, creating noticeable spots on the skin surface. This also applies to the area around his eyes. So to answer the question, he did not likely bleed internally in this scenario, but rather had some small veins burst on the surface of his face and around his eye that led to what you saw.

During: During CPR you are breathing for the person and the normal mechanism that prevents air from entering your stomach and makes it go down to your lungs is no longer active. Therefore the stomach gets bloated as a result of air that is being forced into his airway -some of that air goes into the lungs but some gets into the stomach. It doesn't take much for the stomach to get bloated during CPR as the activity of the entire gastrointestinal tract (including the stomach) stops, and so things begin to back up fairly quickly. I doubt that it was blood from his airway causing all of this, although it could have contributed a bit.

It can give clues: Heart failure is a complicated diagnosis to make and is usually based on history and physical exam findings. An ekg may give clues that point towards a diagnosis of heart failure but can not fully diagnose it. You may be confusing the term heart failure with heart disease. Ask you doctor about further specifics.

It can happen: Unfortunately, sometimes young people also get affected by heart failure. Most often it relates to a viral infection that infects the heart muscle, called "myocarditis". This can lead to substantial heart failure in young patients, and is treated similarly to older patients with hf. However, sometimes the disease can get so bad that heart transplant needs to be considered.

Probably not: My advise to you is that during your early age you decrease all controllable risk factors possible. Keep healthy weight, do not smoke, control chol, bp, exercise, do not drink, control stress.

Change diuretic?: You should ask your physician, if you are on hydrochlorthiazide (h) if you should go to a loop diuretic, forsemide (f). Patients with chronic kidney disease do not respond to h but do respond to f. If that does not work, ask to have an echocardiogram to see if you have pulmonary artery hypertension (p) and what your ejection fraction is. If you have p, there is medicine for that to lessen edema.

No perfect answer: Probably best to avoid combination medicines which contain pseudoephedrine, which many do. This speeds up your heart. Diphenhydramine (benedryl) can dry up secretions. However, it is important to understand that the cough may be due to the chf. Also medications commonly used to treat CHF (ace inhibitors - often end with. Pril) can sometimes cause a chronic dry cough. Talk to your md.

CHF: Congestive heart failure patients usually have structural abnormalities of the heart muscle and can have rhythm abnormalities so yes. Dental care both in the office and at home is important. A the dentist some office procedures and drugs could be problematic. Poor dental hygiene at home could result in infections which could cause valve problems. So dental care is important.
