Sinus congestion: When discomfort in the head or temple area as you describe occurs upon postural changes then, the most likely explanation is that of increased pressure in the head causing expansion of fluid which may be in your sinuses. Jaw pain: Do you have temporal mandibular Joint syndrome (TMJ). Patients have this who grind their teeth, chew gum, frequently eat uncooked veggies, has hx of jaw surgery/trauma. Exam: unfortunately,this sounds like a result of rough sex: I will assume that the choking was consensual; however, I would advise a medical exam - probably soft tissue swelling. Meniere's?: Some of the hallmarks of meniere's include tinnitus (ringing), vertigo (dizziness), and fullness in the ear. There can be some fluctuating hearing loss, and headache (often migraine) can coexist. This would be my first thought for you. Temperal Arteritis: Hello, you should be evaluated by a doctor ASAP (ER or Urgent Care now!). The worse case scenario would be an inflammation of a large artery in the area you describe and the need for immediate treatment with steroid pills. You would need follow up with an ENT doctor to be further evaluated. They might even biopsy the blood vessel. Non-specific: An ear infection can be confined to the skin of the ear canal (external otitis) or fluid or pus in the middle ear space (otitis media). The latter is almost always associated with a hearing loss. Do not strain w/ BM!: Anytime you strain or hold your breath or lift a heavy object, etc. you creating a Vasalva maneuver. So, it puts pressure in your whole system including your CSF potentiallly. Use a stool softener instead or something that relaxes you, versus straining. Also contact your neurosurgeon/neurologist (?) who knows about your CSF leak.
