Some alternatives: I don't understand the meaning of your last sentence; does it mean you have tried insomnia-directed cbt (as well as attention to sleep hygiene) but it hasn't worked? Sleep aids you didn't mentioned include rozerem, (ramelteon) doxepin, and various otc herbal mixtures. Also, withdrawal from some of the meds you mentioned could interfere with sleep.
