Unknown: It sounds as if you are not feeling well at all. Unfortunately it is impossible to know what the cause is without further information. It sounds like you are feeling poorly enough to need a genuine medical exam. Please see a medical provider soon! Hope you are feeling better soon.
Infection: It is possible that you have an ear or sinus infection causing those symptoms - see your doctor to find out.

Everytime I start my cardio workout, I get lightheadedness and mild headache and sometimes moderate pain in my left shoulder. What could be the cause?
Which problem?: Are you asking about exercise intolerance or pain in the shoulder? Unless you have baseline medical problems such as diabetes, congestive heart failure, muscular dystrophy, juvenile arthritis or some other unusual malady for a 17 year old then, I'm sure answers to your questions are very simple. You will just need a little guidance and direction as to what and how you're doing things.

Frequent dizziness, lightheadedness, headaches, confusion and fatigue for 4 months. Occurs in the evenings. Normal brain mri, and other tests. Causes?
Neurologic symptoms: Its not possible to give you a meaningful response. Are you 16 or 96? were you perfectly healthy or had many different medical and psychiatric conditions 4 months ago? You have had a medical work up and we don't know the results of the other tests like blood tests. I cant think of a condition that would just appear in the evenings. It may have to do where you are at that time not the time itself.

Stress reaction: Your symptoms may be related to a heart issue but due to your age and unknown other medical history, this is less likely. Had Costochondritis for a few years now, chest hurts a little. The breathing part is bad. I feel like I can't breathe. I'm lightheaded, what can I do?
Analgesics: If it's really costochondritis, which is simply a form of arthritis, it should improve with simple analgesics. Have you tried, acetaminophen (Tylenol), ibuprofen (Advil/Motrin) or naproxen (Aleve/Naprosyn) in reasonable doses? Also- heat and massage are very helpful. Have you tried a hot tub bath?
Virus likely: These symptoms are nonspecific but could be due to a viral illness which hasn't yet localized to the throat or intestinal tract. Rest, drink adequate fluids and take Tylenol (acetaminophen) as needed. Viral infections should resolve in about a week or so but if symptoms persist longer and especially if there is accompanying fever, cough, urinary symptoms or rash, see your physician for an evaluation and tests.
Since Tuesday just gone have had feeling a lightheadedness when I move. Had headaches too. Now I feel like room spinning.
Contact PCP and get: Referral to ENT for further evaluation for benign vestibular neuritis, or labyrinthitis, as there are specific tests that look at your eyes while your head moves, and seeing if nystagmus is present or not, while the head is in motion. There are tiny hair cells in our inner ear that keeps us balanced, and sensitively connected to eye movements. Treatment depends on what you have. Prognosis is good!
