Diphenhydramine: Aka Benadryl (diphenhydramine) is an anti-histamine that helps block the h1-histamine receptor that leads to itch but beware that it lasts for only 4-6 hours and will cause drowsiness.

Benign side effect: Itching is a common narcotic side effect. It does not mean that you are allergic to codeine. The itching may improve if you take an antihistamine. If the codeine makes you drowsy, choose one of the non-sedating antihistamines available over the counter.

Itching w/ narcotics: Itching is a frequent side effect of narcotic medications and is more likely at higher doses. It is not an allergic mechanism. Narcotics bind to the opioid receptor on mast cells causing them to release histamine, and the result is itching and possibly hives.

Narcotic itch: Great question. Any narcotic (or opioid medication) can cause the side effect of itching. If the itching is severe, you may have to change to a different medication. Many other things can cause itching too. If you continue to have itching after stopping the medication, then you have to rule out other potential offending agents.

Chronic Cough: 65 year old East Asian American gentleman presents with persistent cough he believes started with pharyngitis and developed into severe coughing. Coughing lasted about 2 weeks but now has persisted at night. No complaint of fever, night sweats, hemoptysis. Cough relieved by sitting up. Assessment and plan. Chronic cough that does not go away can be a sign of tuberculosis. You need to have a TB test placed immediately and examine sick contacts, etc. Studies show that the most common causes of chronic cough are upper airway syndrome-postnasal drip (trial of antihistamines indicated), GERD (empiric treatment of GERD indicated) and asthma (trial of bronchodilators indicated). Studies show category C evidence that physicians should order a chest x-ray for non-smokers with a chronic cough (>8 weeks) who are not taking an ACE inhibitor. Since your cough is positional and may be caused by GERD, I would start with an empiric trial of GERD therapy, then antihistamine, then bronchodilators

Allergy: Sound like a reaction to the medication. Itching can usually be helped with Benadryl (diphenhydramine) otc. Just follow the directions on the package. You should also call the doc who gave you the medicine to let them know your symptoms.

I can only guess: Sometimes appetite is increased when anxiety is less and opioids are known to reduce anxiety. While codeine is a "mild" opioid some people are quite sensitive. That's my best guess. Careful though, because I think this sensitivity to codeine might put you at greater risk of becoming addicted to it.
