It can.: But it doesn't have to. When patients come in with "spider bites", basically pimples that keep getting larger, they tend to be staph infections/abscesses. Unlikely: Most common lesion on the back as you described is a pimple or sebaceous cyst. Both of which are localized skin infections/inflammations. A spider bite could occur on back but is less likely. A spider bite could get infected or it could cause tissue necrosis as with the brown recluse spider bites.
