Maybe: You do not mention how large or small it is. A large simple cyst can be painful just as a small one. If it does not respond to treatment and it is still painful, there may not be that many options left other than take it out. ...Read more

Probably ok: Small cyst are not uncommon and can cause pain. The thickening of the uterus can be related to your cycle (it varies throughout the month) at the time of the scan. Rupture is serious: Dermoids can have hair and teeth in them, so are complex on U/S, . The black stuff in the picture is hair. They often grow slowly but rarely rupture. If so, it's like appendicitis and needs immediate surgery.
U/S is a good screen for ovarian masses, but with a "surgical pelvis" CT is done. An early sign of impending rupture on U/S might be fluid in the pelvis, but this is not diagnostic. ...Read more
