Possibly...: From what you describe, the prominence is likely the navicular bone. If you feel from the ankle bone down to it, you'll likely feel a tendon. This is the posterior tibial tendon, which helps support the arch. Sometimes there is an "accessory" navicular present, which makes it even more prominent. If you are having pain, see a podiatrist. Otherwise, it is unlikely you need to be concerned.

Try the therapy: Your doctor wants to be conservative first-good! Surgery is invasive, requires healing time and is not a guarantee. Ask your doctor why he wants to try therapy first. He should be able to tell you expected outcomes and give you a rough timetable for improvement. If you don't improve, you can always do the more invasive surgery.

With chronic pain at this immediate area on the inside of your ankle, have the extra bone removed, but there is conservative care. If surgery is elected, address your flat feet as well. See a podiatrist: It's not uncommon to have accessory bones form in the foot within a tendon, but if it is causing pain, have it examined properly. Based on your description, this may be a biomechanical issue which may have a conservative treatment options such as orthotics but it may require surgery to remove if it is impairing the normal movement. A podiatrist can examine you and give you treatment options.

Maybe: I assuming you had ankle surgery and now have pain and some skin changes. Altered blood can be blue/ black. But it is always a good idea to check with your surgeon. A call may be all you need to do. Seek their input.

Need more info...: And I advise waiting for the official radiologist or nuclear physician report. What is seen on the technologist screen is not of the same quality as the physician interprets. Also, some physicians read them where lighter means more radioactivity, and others where black does, so your description cannot be accurately commented on.

Make sure to have the surgeon explain, in detail, what is expected to be gained from the surgery, as well as, what bad things could happen to your foot as a result of having the surgery. Hopefully, by asking, you will be on your way to being totally informed, before giving your consent to be cut on. (i.e., informed consent).

Varies: There are a number of possibilities such as a ganglion, a calcification in the adjacent tendon, an accessory bone that did not fuse among many other possibilities. I would not hesitate to be seen for an accurate diagnosis.

What exactly: Is your question? Achilles tendonitis and an extra bone. The injury did not cause the extra bone. It could of caused the tendonitis but you did not share what the injury was. Are you asking what to do now? Physical therapy is good for the tendon, sometimes docs advocate immobilization, others like eccentric excercises. Heel lifts may be helpful. Or immobilization in a cam walker type boot.
