Depends on his: Symptoms. If symptoms resolve after treatment with antibiotics, then medical treatment without surgery should be ok. If symptoms recur, or pain, fever, bleeding, or diarrhea, intervene, then surgery might become necessary. Most importantly, make sure that this is not colon cancer. Diverticulosis: Diverticulosis is essentially caused by pressure. Straining at stool causes pressure within the colon, and that pressure seeks to escape through weak points in the intestinal wall. When escaping, the pressure takes some of the intestinal lining along for the ride causing pouches (diverticuli). No infection.: Diverticulosis by itself is not dangerous. Just requires a high fiber diet in the long run. Diverticuli can bleed or become infected but the lack of an acute process on the CT tells us that there's nothing to worry about right now. ...Read more

Many possibilities: Its normal to be scared when an ultasound shows a possible mass and further imaging is needed. It is important to get the ct in order to help the doctor to make a diagnosis. Often times the ct scan shows there is no finding to support the ultrasound findings. But it is important to get the study. Gallbladder?: Might be worth checking out your gallbladder-- ultrasound is a better study for gallstones, and if there aren't any, then you may need a cck-hida scan to evaluate gallbladder function. Even without stones, if the function is abnormal you can have similar symptoms- right upper quadrant pain, radiation to shoulder, fatty food intolerance, flatulence etc. ...Read more

Possible but rare: Yes diverticulosis is benign, has distinct appearance from CA on CT for a trained radiologist will read as accurate as reading finger prints, some times in between diverticula the wall may be thickened, may have mucosal overgrowth will mimic a early cancer, then colonoscopy indicated, at 39 cancer is not common but age is not exception for CA, ask again for referral. ...Read more

Absolutely: Often diverticulitis has a chronic element, which allows adenopathy.
Earlier (acute) signs are the "misty mesentery" &/or smudged pericolonic fat which is inflammatory change in the fatty tissue that suspends the sigmoid. Also we look for free fluid and esp. Free air (perforation).
Care must be taken to exclude inflammatory change near a cancer so F/U scan/colonoscopy is often done after a while. ...Read more

Barium is a laxative: What do you mean by "irritation"? Abdominal pain? Increased output? Blood? Barium, for all its use in imaging, is also a laxative & often used as an enema. It will force the stool out of the intestines & colon as it makes its way out. Therefore, "irritation" of the colon (depending on how you define it) isn't uncommon after a barium-contrast CT. Perhaps...but: Diverticulitis is an inflammatory condition of the colon. Attacks can occur once or repeatedly and usually are treated with antibiotics, reserving surgery for those with complications or for severe, repeat attacks. A ct while you hurt should show inflammation, but a ct just before the attack can be normal. Persistent pain is a good reason to see a surgeon, even if you saw one before, to be safe. ...Read more

I'm afraid: That the first may have been a situation leading to the second ct scan result. The fistula has closed leaving an abscess "in its wake"......check this out ASAP with your GI doc for possible surgical referral.

At age 61: Would suggest CT scan NOT SUFFICIENT to rule out bowel disorders. You need a screening Endoscopic exam....by a qualified Gastroenterologist. ..Hope this is helpful
