Unclear: If your are referring to a para esophageal hernia, which is a type of hiatal hernia, it could be responsible for the pain. Sometimes the colon is herniated in to the chest as well, and this could be responsible for the diarrhea, but it is less likely. Went to the ER with a lot of blood in my diarrhea. They did a CT scan w/ contrast. There is mild diffuse wall thickening of the descending colon and parts of the sigmoid colon. Also noted was a umbilical hernia 1.3cm. What could cause the blood?

Probably not all: Probably not all of those symptoms are related to an umbilical hernia. Most umbilical hernias are small and do not cause that much discomfort, naseua, fatigue, weight loss, etc. If it is really large, it can start having some of those symptoms. Not normal.: C.difficile enterocolitis is a possibility. Often peri-operative antibiotics adversely affect the bacterial content of the colon resulting in an overgrowth of a bug called clostridium difficile. This causes inflammationc of the colon lining with resultant severe diarrhea. Stool culture is needed to make confirm the diagnosis. Possibly: Spigelian hernias are notoriously difficult to diagnose since the hernia is in the inner muscle layer of the abdominal wall; since there is intact fascia above the hernia, one often cannot see or feel a bulge. Not from the surgery: It is very unlikely that these current symptoms are related to your surgery. By far the most common cause of diarrhea is viral infection (enteritis, or gastroenteritis if it involves nausea/vomiting). With any diarrhea, weight loss, dehydration (feeling like you're going to pass out when standing), or fever lasting > 1 day would be cause for concern.
