What is the safest way to replace fluids in a dialysis pt who is having severe diarrhea secondary to opiate withdrawal?
Couple of Choices: You could manipulate the dialysate solution and or vary ultrafiltration to normalize the electrolytes and fluid balance or you could try to manage the withdrawal symptoms. This could be tricky in a esrd patient as the most common rx is Clonidine which can abruptly lower bp. A good idea,, : This is probably a good thing for him as he has fluid in his lungs and a high creatinine. This and the known history of diabetes likely means he has renal failure. My dad is in the ICU for acute pneumonia. Has diabetes and partial kidney failure as well. Will dialysis help in recovery and in reducing lung fluid?
It depends: Dialysis would be needed for severe renal failure that is causing significant metabolic abnormalities or fluid overload. It may not necessarily help in cases of mild or moderate renal failure and pneumonia. It definitely wouldn't help remove the inflammatory fluid from a pneumonia. Check liver: This sounds like a very serious problem. The yellow eyes suggests jaundice and liver dysfunction.
