If you block/decrease dht, you could potentially increase e2. Too much e2 in men leads to moobs (man boobs), nipple sensitivity, gynecomastia, loss of libido, erectile dysfunction, etc.

Usually, CAREFUL.: Usually but not always. There may be early effects in men with reduced potency and libido, and some with gynecomastia. Occasionally the sexual effects are permanent.

Not great: There is already data in regards to topical topical finasteride and topical fluridil, both of which are topical antiandrogens. The data isn't impressive at all. Remember, anything that can be absorbed into the skin can be absorbed into the body (systemically) though it doesn't mean that it happens in large quantities. If you're that interested in treatment consider sticking more main stream.

Sometimes: Some men may experience breast growth while taking a DHT blocker, especially if on the drug for a prolonged period. Libido can also happen. If this happens stopping it will regress problems. Hope this helped! @christianassad

I have a high DHT level. Would a 5a-reductase inhibitor be a better choice for treating my BPH (storage, not voiding, is prob.) than an alpha blocker?

See below: 5 a-reductase inhibitor will decrease the conversion of testosteronte to dht. It takes 3, &sometimes up to 6, months to show demonstrable effect. Alpha blockers start showing effect within a few hours. If I were treating you I would start you on Flomax (tamsulosin). If improvement is less than desirable I would add Proscar (or other bands in same groups).There is also a combination pill which has both (Jalyn).

Big guns: Almost all acne is hormonal. The 5-alpha reductase (dihydrotestosterone) inhibitors actually have found some use in managing acne. Your best choice might be topical spironolactone. If your acne has not responded to antibiotics, you're a tough case. If you're offered Isotretinoin instead, consider it. Best wishes.

Age 80: I am not sure I understand your question but at your age (if I read it correctly) you may be having side effects from your own testosterone - such as an enlarged prostate. In that case you must see a Urologist. I cannot think of any reason to consider an estrogen inhibitor for you.

No difference: Side effects form the medication are no different whether given as short- or long-acting. You may feel the effect of the medication more with short-acting forms (such as lightheaded with BP meds as it brings the pressure down). The most common side effect from ace is cough which occurs with either form.

Depends: I agree that twice daily dosing in clinical trials has somewhat better effects on the neuroendocrine system, however many pills are not safe to split--especially extended release formulations. It depends on the manufacturer. Aceon, mavik, (trandolapril) and univasc may be splittable ace inhibitors if you are trying to save money, but please consult your doctor first! Don't split non-scored pills!

Depending on Pt: Each pt is different. People respond to different meds differently. Some pts ace inhibitor works very well. Some pts it does not. We have to try one med at a time and see if it works. If it doesn't will try next option.

Question for your Dr: Ace inhibitors are generally a once daily medication, but can be taken at lower doses more frequently. Nocturnal dosing may help avert undesired side effects. Before breaking up a pill in half and taking it twice a day in hopes of decreasing side effects, you should discuss with your dr. A lower dose or a different class of medications may better serve you depending on what your side effects are.

Very few for all.: My experience and the published literature on comparative side-effects say they are about the same and not very common. My bias is for Humira and against remicade (infliximab). As far as response and efficacy, most recent studies say that they are equivalant in rheumatoid arthritis.

PPI side effects: There are concerns that reducing stomach acidity may be contributing to clostridium difficile-associated diarrhea. There also may be a link between long-term use of ppis & osteoporosis. If you develop resistant diarrhea or are at risk for osteoporosis, consult your fp for further evaluation.

Depends: You must mean you have protein in the urine. We do give ace inhibitors or arbs for that condition especially in diabetics to protect their kidney. A good medicine does not mean it is good for everyone who can't tolerate it. Some can get worse too. Ask your doctor, have BP and hr examined while on the medicine or off and see if it is the cause of your problem and do what is most appropriate.
