Rupture is serious: Dermoids can have hair and teeth in them, so are complex on U/S, . The black stuff in the picture is hair. They often grow slowly but rarely rupture. If so, it's like appendicitis and needs immediate surgery.
U/S is a good screen for ovarian masses, but with a "surgical pelvis" CT is done. An early sign of impending rupture on U/S might be fluid in the pelvis, but this is not diagnostic. ...Read more

Although they may hurt the best treatment is to take Ibuprofen for the pain and to wait to see if it goes away. Will likely go away: Physiologic simple cysts that are under hormonal influence will come and go often, sometimes by a slow leak, sometimes by a rupture and sometimes by your body essentially absorbing it. Would not happen US: Should not wait that long should come out now a nd u need blood work to see if it is malignant or premalignant which the blood work can give u an idea but the risk of leaving it in is that it could twist or rupture and if it is cancer (hopefully not or precancer) it needs to come out that is why I get pts coming down here from canada. ...Read more

Most likely: Ovarian cysts are common findings. Most cysts are 'simple' (benign appearing). Studies show than most cysts will resolve with time and surgery is not necessary, even for cysts as large as 10 cm. Unless a woman is having symptoms such as pain suggestive of ovarian torsion, most cyst may be manage with serial ultrasound every 3-6 months to evaluate for resolution or increase in size. ...Read more

Information: Is the septation 5cm or the cyst? Since you know you have endometriosis, it could be an endometiroma. However, septated cysts are often monitored closely, with frequent ultrasounds and follow up until regression which can take up to a year. If they had atypical characteristics you might want them followed but most of the time they would be either developing or regressing follicles.
