See a dermatologist: It is difficult to make a proper diagnosis since I am not there to see the condition. I recommend seeing a dermatologist so that they can make a proper diagnosis and give you something to get rid of the condition or relieve symptoms.

It would be hard for: Us to determine the cause or the treatment. Don't know what your medical workup showed or your diagnosis. Sometimes acupuncture can be used to treat symptoms even if cause is not known.

Either: Your primary care physician should be able to provide first line treatment for you. An allergist will help more in finding the triggers of your eczema. A dermatologist will be more help in managing your skin.

See a neurologist: When pain is due to a functional abnormality of the nervous system it is called neuropathic pain. Neuropathic pain can occur spontaneously, is often exaggerated and it is a constant, dull, aching, burning pain. See a neurologist for evaluation.

Other things: Make sure his liver function is ok. There are rare liver /gall bladder problems that can cause uncontrollable itching. In addition, there are some behavioral conditions that do the same. This is despite the suggestion of an allergic problem.

Itching: A tough one. Look up a leap test from signet labs in case it is a food intolerance not an allergy. If the ANA is negative and parsite tests are negative try a homeopathic like urtica urens.

Depends: If the reaction is a drop in blood pressure, hives and wheezing with fire ant bites, an allergist can desensitize the individual to decrease the likely hood of a repeat severe allergic reaction. With a local reactions only either can treat the reaction.

Allergist vs Dermatologist: Any competent specialist in either field would help you, now that it is there for 4 months it became chronic, there are several other antihistamines than loratadine to deal with your case in different dose regimens, beside other medicines if needed, more important is history, examination and probably some investigations, good luck

Depends: Both specialities deal with allergic skin conditions: atopic eczema, contact eczema, urticaria of different forms, if the allergic condition is affecting more than one system, skin and respiratory tract for example, allergists deal with both, although there are no sharp lines, except for special expertise needed in either specialty, such as desensitization practiced by allergists

Depends: It depends on what is causing the rash and how it looks.... Sometimes your regular medical doctor (internist/pediatrician/fp) can help you decide. If it is red, itchy then an allergist is usually a good idea. If it's dark then a dermatologist is a good idea. This is just a general "rule of thumb"...

Start with PCP: Would recommend starting with your PCP in trying to sort it out. Generalized itching with no rash may be caused by medical conditions. Keep a symptom log, including physical activities, showering, food and drinks ingested, etc.
