Cyst likely: Cysts contain watery fluid & no blood vessels thus no blood flow. Lining of cyst is very thin & has blood vessels & flow but flow insufficient 2B detected by U/S. Suggest consult your "anxiety" Dr. If it's not adequately controlled. Good Luck.

Varicocele: Is highly unlikely that varicoceles cause severe pain, there is usually something else going on. The cyst on the right testicle is probably a spermatocele and mostly treated conservatively with nsaid's, as a last resort, surgery. I would see a urologist and see what he recommends.

Spermatocele?: You are likely describing something called a spermatocele. This is a cyst at the head of the epididymis containing sperm. Unless they are causing symptoms they are left alone. They do not interfere with fertility. Cancer will be a hard area felt on the testicle. Best to see a physician to be sure it's a spermatocele.
