Go see your doctor: Post-tussive emesis is vomiting after coughing hard enough. In & of it self, vomiting after coughing really hard isn't too big of a deal as long as you're otherwise breathing & swallowing ok. However, coughing or vomiting up black phlegm can be concerning for blood, especially in a smoker. So go see your doctor or go to urgent care/emergency department if necessary. Possibilities...,: You may have acute bronchitis from either a viral or bacterial infection. You may also have bronchospasm contributing to this cough. See your doctor for diagnosis and treatment. You really need to quit smoking asap to decrease your airway inflammation. If this cough has been present for more than 3 weeks, chronic bronchitis needs to be ruled out as well. Chronic cough: Chronic cough is not normal. The 4 major causes of chronic cough are asthma, chronic sinus problems, post nasal drip from allergies and stomach acid reflux. See your doctor!!: Blood in sputum should always be evaluated. Depending on your age, there is always concern that blood may represent something serious. It is most frequently just inflammation from smoking, but more serious conditions, particularly cancer (in older individuals with longer smoking histories) must be ruled out. Stuff going up/down: Sounds like acid reflux to me. But upper respiratory issues (sinusitis, allergies) causing mucus drainage down the throat or acid reflux up the throat can commonly result in those symptoms.
