In daily adrenal insufficiency supplementation, which is worse: one peak of cortisol in the upper 30s or low 40s, or three peaks between 16 and 22?
Not studied: Hi. I hope very much you were correctly diagnosed. Assuming so, I do not know why your doc is even checking peak cortisol levels...well known and shown to not be valuable or predictive of problems. Use the lowest hydrocortisone (= cortisol) or prednisone dose needed to prevent hypo adrenal symptoms. Blood cortisol levels are not helpful. Good luck! ...Read more
Depends: When it was done. Cortisol levels are higher in the am (15.3 would be normal) then in the pm (this level would be too high for the pm) ask your doctor why he/she drew it in the first place. ...Read more
Acth level is less than 1.0 with normal cortisol (8). Does this indicate any issues with thyroprotein level of 3? Thanks
Adrenal stim test. Baseline cortisol at 0930 is 17.3, at 1000 is 18.9, at 1030 is 20. Ref range is 3.7-19.4. What causes wnl baseline/min elevation?
If cortisol was 17 within the first 3 hrs of waking, non fasting, then 33 an hour after stim, is that normal or borderline?
More info, please: Hi. In the units we normally use, those would be low. What is your doc testing them for? Were they drawn the morning after taking a dexamethasone pill? If you, I'd say you suppressed normally. If it wasn't dex suppression test, you may be adrenally insufficient; if so, it would look like a pituitary origin. Provocative testing would be required to confirm, such as insulin tolerance testing. ...Read more
Do these cortisol stim results appear normal? Taken at 10 a.m.:
ACTH - 7.6 pm
Base - 4.3 ug
30 min - 21.3 ug
60 min - 28.2 ug
ACTH stim: It appears that the baseline value is low. there is then a brisk response over 21 mcg cortisol at 30 minutes. It increases further at 60 minutes. this pattern can be seen with some forms of pituitary suppression, most commonly taking exogenous glucocorticoids. Cortisol levels: Cortisol varies throughout the day. It is generally higher in the am vs the pm, but it is spiking up and down all day. Normal walking would not particularly elevate it, but it goes up/down all the time anyway. The more important question is why is cortisol being measured? Depends on dose: The reported half life of IV cortisol is about 1 hr for lower doses, 2 hr for higher doses. Cortisol given intramuscularly, subcutaneous, or in the spine will have a different half life that depends on how it is absorbed into the blood. ...Read more
