Yes: Simvistatin at high levels has been shown to have a significant risk of muscle aches/pains and possible muscle damage which can lead to a lot of other problems. Itraconazole raises Simvistatin levels. If you are on a normal dose of Simvistatin the itraconazole could push your levels into the danger range. If you are going to be on the itraconazole for a limited time, you may want to be off Simva.

Too little salt can dehydrate.

Thiazides in CKD: Thiazides are not contraindicated in renal failure for any medical reason. They are not used as they are ineffective of making patients urinate. Patients with chronic kidney disease are placed on loop diuretics which are able to evoke a diuretic response in patients with it. Patients on dialysis are given these drugs when they still have residual renal function so they can eliminate urine.
