9mos concussion, pcs. Hit head again 2mos ago. Evaluated neur/psych, ct/mri normal. Invasive desire to hit head. Dizzy when the feeling comes. Wtf?
Postconcussive: A postconcussive syndrome can bring about any other psychiatric syndrome including an impulse control disorder nos. Seen from this perspective, medicinally one might expect benefit from Hydroxyzine dosed as tolerated between 10 mg daily up to 25 mg PO qid. If you also have posttraumatic stress disorder symptomatology, there is always the possibility of a self-directed anger coming back at you in the form of a repetition compulsion.Either way, psychotherapy along with medication would certainly be the way to go.Low-dose antipsychotic interventions are sometimes also helpful in such conditions but i would certainly give Hydroxyzine a trial run first if you were among my patient population. It's Possible: To get concussion without actually striking head but if you really were driving that slowly it is unlikely. Testing is needed: This is not just a concussion, since you passed out first. This history is important. Some of the conditions that cause loss of consciousness can cause exercise intolerance.
