An ultrasound: Would confirm that this simply represents an ovarian cyst. The chance of the finding you describe representing melanoma is almost zero.

Need Fertility Exam: I would refer you to a fertility specialist. Certainly a complex ovarian cyst can be secondary to endometriosis which can adversely affect fertility. Ovarian cyst resulting from polycystic ovary syndrome – a constellation of small cysts in the ovaries, irregular periods, elevated androgens – can contribute to fertility problems. A fertility exam is required for specific answers.

Low degree of concer: Most common cause of such benign cysts are functional cysts that are very common in reproductive age females. Unless, presenting with symptoms (pain, nausea, vomiting, fever etc), your physician may opt to follow them with a repeat us in 3-6 months. Unless you have strong family hx of cancer, or personal hx of cancer, there is a low level of suspicion for ovarian cancer.

Fibrin in cyst: Ovarian cysts frequently will have stuff inside. When they do not, it is a simple cyst. When there is bleeding in to the cyst, they become complex. This causes all kinds of findings on ultrasound. Blood clots look like clumps and fibrin can cause stranding. All usually clear up

Painful?: It looks like you have a GYN who is doing all of the right tests. So what does she/he recommend? In my practice, I recommend surgery for cysts this size that are painful. Cysts that are not can get serial ultrasounds. Most of my patients with cysts this size have opted for surgery since pain from these masses can escalate if they enlarge. Deep breath: Take a deep breath, no need to panic yet. It is 100% appropriate to follow a 62 Y.O. W/ a complex cyst, some turn out to be cancers but many do not. Follow-up ultrasound is critical, but may turn out to be nothing. Blood testing tumor markers is sometimes helpful but not always, and should only be done with a doctor who understands markers and what they mean. Please don't panic and best wishes!

Follow-up required: In a patient of your age, it is likely self-limited and very well could represent a hemorrhagic cyst. However, a repeat ultrasound 6 weeks following the first is suggested to document resolution or decrease in size and to re-evaluate for patency of the ovarian artery.

Hard to say..: These may not be related to each other. Many things can cause pelvic pain, if you have large ovarian cysts (>4cm) they may be the cause, but smaller ones less likely to cause pain. Low prog levels (&small ov cysts) are both normal during certain phases of menstrual cycle. But low prog can cause mood sx, anxiety, sleep disturbances. If you have these natural Progesterone tx may help.

OVARIAN CYST: Assuming that this ovarian cyst is an unruptured follicle, ovulation will not take place (the egg failed to break the follicle and be released). If this cyst has persisted for some time, it may not have an effect on your ability to become pregnant. Many cysts break, and then normal cycles can resume. Check with your health professional to make sure the cyst is simple and no surgery is required

Depends on the: Nature of the cyst. Usual ovarian cysts do not produce hcg. A cystic germ cell tumor would need to be considered if an ovarian cyst is associated with elevated hcg.

Thyroid disease: Any thyroid abnormality, whether excessive thyroid hormone or less than normal can affect a woman's menstrual cycle, but either does not generally cause ovarian cysts. Your B12 level will not cause an ovarian cyst. Ovarian cysts are spontaneous, and at 56, may require surgery to diagnose the cause. See your physician for advice soon.

Low grade temp: Your temp is considered normal and probably has nothing to do with your surgery. If you are concerned see your doctor for a complete medical workup.

Delayed menses: Periods are often delayed after pregnancy. A 'lazy' pituitary and an ovarian cyst are common causes that will resolve on their own. Uterine scar tissue is a rarer cause. An ultrasound and a blood test can reveal the reasons, and would be appropriate after 3 months.
