Sphincter muscle: At the end of the duct, just at it's entry to the intestine. There is a circular muscle that closes off the duct when bile is not needed to digest foods. Since the liver never stops making bile, it will build up inside the bile ducts and stretch them out when a gallbladder has been removed. A stone could block the duct but will produce symptoms. Colicky pain: Gallstones/sludge cause symptoms referred to as biliary dyskinesia (not the other way around). Large stones can get stuck in the common bile duct but that usually causes severe pain and sometimes jaundice. Dyskinesia is pain that comes and goes, especially triggered by fatty food. The symptoms can be controlled with diet for a while but the best treatment is gallbladder removal. ...Read more

Misinterpretaton: "Transaminitis" refers to elevation of the enzymes ALT and AST that results from inflammation of the liver. Blockage of the common bile duct from any cause can elevate AST & ALT. If there's no permanent liver injury after removal of the stone, ALT & AST will gradually return to normal. SEVERE liver failure can cause toxicity to the brain but it's not from" transaminitis".
