It releases a toxin that causes necrosis and ulcers in the colon. The infection may be acquired in the hospital and sperad by hospital, on their hands, from one patient to another.

Megacolon
Most healthy patients with c.diff get treated and do well. Unfortunately, older and more immunocompromised patients are susceptible to toxic megacolon from c.diff. If you do have this condition you should see a doctor and get appropriate antibiotic treatment.
