Is topamax (topiramate) an inhibitor or inducer or cyp2c19? Weak or not, i'd still like to know. Thank you kind doctors.

Is topamax (topiramate) an inhibitor or inducer or cyp2c19? Weak or not, i'd still like to know. Thank you kind doctors.

Weak inhibitor: Topamax (topiramate) is a weak inhibitor of the cyp2c19 enzyme. This enzyme is responsible for the metabolism of several drugs including Phenytoin (dilantin). Therefore, the concomitant use of Dilantin and topamax (topiramate) may be associated with increased Dilantin levels. This is probably more evident with topamax (topiramate) doses that exceed 200 mg/day. I hope this helps! ...Read more

Treating Hepatitis B: The answer to this question lies with the doctor who is prescribing you viread (tenofovir). Viread (tenofovir) can be very hard on one's kidneys. If there is any worry about your kidneys then this creatine is probably not a wise decision. ...Read more

After Heart CABG+ligation LAA, better taken warfarin 2mg + clopidogrel 75mg or rivaroxaban 20mg + clopidogrel 75mg ??

After Heart CABG+ligation LAA, better taken warfarin 2mg + clopidogrel 75mg or rivaroxaban 20mg + clopidogrel 75mg ??

Complex issue: While warfarin has been around longer with an excellent safety record, the new kid on the block, rivaroxaban, also appears to be possibly safer. A major difference, patients don't have to get their blood tests every 7 to 30 days with rivaroxaban. That in itself makes rivaroxaban a nicer alternative. ...Read more

Does heterozygous for MTHFR gene mutation affect pregnancy?, and what about taking synthetic folic acid supplement?

Does heterozygous for MTHFR gene mutation affect pregnancy?, and what about taking synthetic folic acid supplement?

Controversial...: Some studies have shown an association between the MTHFR gene mutation and recurrent miscarriages but this is controversial. Aside from this possibility, it doesn't cause problems with pregnancy. If you have this mutation, its a good idea to take folic acid, vitamin B-12 and Vitamen B-6. Aspirin is also sometimes recommended for women for who have had recurrent miscarriages. Best wishes! ...Read more

HmG 10.7- ferritin 6- hematologist said heavy periods &taking high dose nexium (esomeprazole) which stops iron absorption. Taking Ferrex 150 BID-how to fix Fast?

Long term: The fact that you became anemic means you lost all of your body stores of iron and it will take about a year to build up the stores, even after your hemoglobin becomes normal. You may try taking your iron pills three times a day. Take these with meals. For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. Practice safe sex. ...Read more

Heterozygous for MTHFR, have high Protein C & Fact, VIII. Taking 3mg methylfolate for 3 mo, homocysteine normal . Reason to follow up w. hematologist?

No: Forgive my frankness. Most of us pathologists do not believe in this MTHFR screening being promoted by a lab to physicians worried about liability. A majority of folks test "abnormal" and are then offered questionable vitamin supplements. Reference ranges arbitrarily put several percent of healthies outside; high protein C and factor VIII are not worrisome especially if you feel well. ...Read more
