Are you right handed: That may be the most simple answer. If you are right handed, you perform more movements, carry more things, and generally work out your right arm and shoulder more than your left, which could easily account for some muscle asymmetry, right side larger than left.

Diagnosis: It really all comes down to the specific diagnosis. Long term narcotics would not be a good idea. Most common cause is an undeyinh structural problem in the spine such as a "pinched nerve" or a symptomatic facet joint. Other possibility us cervical dystonia.
