No: It will not go away with medication. The cause of the cyst is endometrial glands on the ovary which need to be removed. With the size of the cyst it can rupture and cause and emergency situation. Best to treat with surgery in a timely manner.

Surgery best: A "chocolate" cyst occurs when endometriosis attaches to the ovary and bleeds into it. The old blood collects and forms a cyst. Your previous surgery did not cause this, and medical treatment of endometriosis will not get rid of it. Surgery is the best option.

Not usual: Ovarian cysts form every month in healthy young women. Normal ovaries may be filled with small cysts (known as functional cysts). Once in a while, a cyst will become large and may cause pain due to pressure on surrounding tissues. Many ovarian cysts resolve on their own in 3-4 weeks, but an enlarged cyst can rupture causing significant pain. Hopefully you will not need surgery again.

Hormonal Suppression: Excisional surgery is only a temporary fix for chocolate cysts related to endometriosis. Unless your ovaries are being removed (and you're too young for that), one thing to try would be hormonal suppression; hibernating your ovaries, via use of continuous oral contraceptive pills or an injection called Lupron (leuprolide) that shuts down your cycling. This can potentially quiet down any active endometriosis.

Varies: Certainly pregnancy should be the first goal. If you have not completed childbearing that should be the main focus. Having surgery on the ovary can lead to worsening infertility. Certainly you need a fertility specialist to coordinate any care since you need to be fully counseled on whether it is possible to have kids, how difficult it will or won't be, . And what surgery will help versus hurt you.
