Look in the ear first: You can buy a cheap otoscope at the pharmacy and looked directly in the ear at the eardrum. For a three-year-old I would wonder first of all do they have an infection. Ear pain is extremely common in childhood and although it is distressing, is usually caused by a minor infection that will resolve on its own.
