CT - when?: It's reassuring to know that your CT was normal but was it done for this illness (recently?). Your symptoms can be due to muscular pain, pleurisy or pericarditis. If you have fever, you need repeat chest x-ray, blood count and echo. Otherwise, I'd try an analgesic - ibuprofen or naprosyn (naproxen). ...Read more

Muscular: With a negative CT scan, I think we can be assured that your pain is muscular or in the costochondral joints which attach your ribs to your sternum (and would not show up on CT). When you breathe, you're using intercostal muscles and flexing these joints. If you search diligently, you may be able to find trigger points by compressing different areas when they hurt. R/O PERICARDITIS: You may have inflammation of the heart=PERICARDITIS vs inflammation of the lungs =PLEURITIS. Myocarditis is much more serious and can be suggested by an abnormal echo. Often they are post viral. They tend to resolve with rest/hydration /NSAID (indocin) if it persists as course of prednisone may be needed. You need a prompt evaluation EKG/CXR AND LABS. Rib dislocation: Could be a rib dislocation or possibly costochondritis (inflammation of where the ribs and cartilage meet). Working too hard: You are likely exceeding your maximum exercise heart rate. Exercise heart rate should be 80% of you age maximum. If you are 28 y/o male, that number should be 150-155 beats per minute. At 170+ bpm, you are working your heart too hard and it is telling you (your symptoms described above) to turn it down a notch.
