Neck if not heart: Any time one experiences chest pain with Jaw, shoulder and arm involvement, ruling out a cardiac cause is paramount. Once done though, your cervical spine can be the underlying trigger for most of these complaints. Chest pain, jaw pain, leftarmpain, palps. All normal tests so far 2012/2013. 11 months symptom free taking Diltiazem. Now returned. Angiogram needed?

Probably not...but: It maybe necessary given your persistent symptoms and the anxiety they are likely causing. Remember non-invasive tests are not 100%. Angiography is the good standard in ruling out coronary artery disease. I suspect you could be having coronary artery spasm that can't be picked up on angiography usually (just my 2 pennies). Very Hard to say: If you've had testing to rule out major problems, then it could be anxiety, over exertion or other lesser problems. What brings on these symptoms? Occur after eating? Do you have them constantly or if occasional, then try to keep a log, write down what you were doing prior to feeling these symptoms, possibly you are doing something, consistently that can be changed to stop these problems. ...Read more

Yes: Neck and jaw pain without chest pain would be considered an "atypical" presentation but it does happen. Remember that a heart attack can (and does in 30-50% of cases) have no pain. Other symptoms that commonly accompany heart attacks are: breathlessness, cold-clammy sweat, nausea and sometimes vomiting.
