Internist or an ER: You need lungs evaluated. In an ER your Oxygen saturation and vital signs can be eval. On Physical exam should be able to detect how well lungs fill with air. You will also get an XRAY of your chest. An ER is expensive. Internist (or Urgent care) will also have a saturation meter and should be able to detect how well lungs fill. Wow: Do you smoke? Obviously if you don't smoke something else is going on. Unusual at age 20 but with your co morbidities especially diabetes i think you should be under the care of a cardiologist ; an endocrinologist and make sure they consult with each other.
